老年女性髋关节炎及各病种死亡率的风险研究

目的：评价老年髋关节炎患者各种疾病及特殊疾病的死亡率，明确疾病发生的调节机制。

方法：数据源自一项美国位大于65岁白人女性的骨质疏松骨折的队列研究。分析样本，有髋关节平片基线（n=），8年（n=）.死亡率统计始于年10月，医院的死亡证明判定。OA患者的髋关节炎的影像学表现（RHOA）至少有一侧髋关节Croft评分≥2分（关节间隙狭窄或者有骨坠形成，加上至少1项其它影像学表现）。

结果：平均的随访时间为16.1--+-6.2年，患者影像学随访基线年限8年的为8%，个别为11%。

累积发病率（研究期间发生的死亡比率）为67.7%的全因死亡率，26.3%死于心血管事件，11.7%死于癌症，1.9%死于胃肠道疾病，27.8%死于其它病因。RHOA与全因死亡率相关，全因死亡率的风险增加（风险比率1.14[95%可信区间1.05，1.24]），心血管疾病死亡率（危险比95%[（风险比率1.24[95%可信区间1.09-1.24]），校正身体质量指数，教育调程度，吸烟，健康状况，糖尿病和中风。这些联系部分可以被人体生理机能的调节所解释。

结论：通过16年的长期随访，患者髋关节的影像学表现和白人老年骨关节炎患者的心血管死亡率及各种原因所致的死亡率密切相关。骨关节炎患者的生活管理、规律治疗有益于降低患者的死亡率。

Results.Themean6SDfollowuptimewas16..2years.Thebaselineandyear8prevalenceofRHOAwere8.0%and11.0%,respectively.Thecumulativeincidence(proportionofdeathsduringthestudyperiod)was67.7%forall-causemortality,26.3%forcardiovasculardisease(CVD)mortality,11.7%forcancermortality,1.9%forgastrointestinaldiseasemortality,and27.8%forallothermortalitycauses.RHOAwasassociatedwithanincreasedriskofall-causemortality(hazardratio1.14[95%confidenceinterval1.05–1.24])andCVDmortality(hazardratio1.24[95%confidenceinterval1.09–1.41])adjustedforage,bodymassindex,education,smoking,healthstatus,diabetes,andstroke.Theseassociationswerepartiallyexplainedbythemediatingvariableofphysicalfunction.

Conclusion.RHOAwasassociatedwithanincreasedriskofall-causeandCVDmortalityamongolderwhitewomenfollowedupfor16years.Disseminationofevidence-basedphysicalactivityandself-managementinterventionsforhipOAin
